The vaccination process follows the same rules as for older teenagers. Individuals under 18 years of age are administered the <strong data-start=\"458\" data-end=\"480\">Comirnaty (Pfizer)<\/strong> vaccine, which has been approved for this age group by the <strong data-start=\"540\" data-end=\"575\">European Medicines Agency (EMA)<\/strong>.<\/p><p data-start=\"580\" data-end=\"759\"><strong data-start=\"580\" data-end=\"594\">Important:<\/strong> For minors, the <strong data-start=\"611\" data-end=\"654\">pre-vaccination screening questionnaire<\/strong> must be signed by a <strong data-start=\"675\" data-end=\"703\">parent or legal guardian<\/strong>, who must also be <strong data-start=\"722\" data-end=\"756\">present during the vaccination<\/strong>.<\/p><p data-start=\"761\" data-end=\"805\">A child can be registered for vaccination:<\/p><ul data-start=\"806\" data-end=\"1075\"><li data-start=\"806\" data-end=\"941\"><p data-start=\"808\" data-end=\"941\">through the <a href=\"https:\/\/www.gov.pl\/web\/szczepimysie\"><strong data-start=\"820\" data-end=\"850\">online registration system<\/strong><\/a>,<\/p><\/li><li data-start=\"942\" data-end=\"1032\"><p data-start=\"944\" data-end=\"1032\">via the <strong data-start=\"952\" data-end=\"990\">nationwide, free, 24-hour helpline<\/strong> of the National Health Fund at <strong data-start=\"1022\" data-end=\"1029\">989<\/strong>,<\/p><\/li><li data-start=\"1033\" data-end=\"1075\"><p data-start=\"1035\" data-end=\"1075\">or <strong data-start=\"1038\" data-end=\"1072\">directly at vaccination points<\/strong>.<\/p><\/li><\/ul><p data-start=\"1077\" data-end=\"1253\">Individuals who choose the <strong data-start=\"1104\" data-end=\"1144\">\u0141ukasiewicz \u2013 PORT vaccination point<\/strong> can book an appointment by calling <strong data-start=\"1180\" data-end=\"1196\">71 734 74 00<\/strong>, Monday to Friday, between <strong data-start=\"1224\" data-end=\"1251\">1:00 p.m. and 4:00 p.m.<\/strong><\/p><p data-start=\"1255\" data-end=\"1426\">The <strong data-start=\"1259\" data-end=\"1299\">\u0141ukasiewicz \u2013 PORT vaccination point<\/strong> is located on the <strong data-start=\"1318\" data-end=\"1335\">Pracze Campus<\/strong> (147 Stab\u0142owicka Street).
