    The aim of the project is to develop the RESET (Ready for Safe Cancer Treatment)<\/strong> tool \u2013 a comprehensive solution designed to optimize care for oncology patients. Its primary objective is to ensure a safe and timely discharge from the oncological surgery ward and to reduce the number of postoperative complications, prolonged hospital stays, and rehospitalizations (PH\/RH).<\/p>

    RESET will be developed based on a detailed analysis of adverse events and medical parameters, and its effectiveness will be evaluated by assessing the potential health, financial, and organizational benefits resulting from its implementation.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

    Three Main Areas of Benefit:<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

    • Health:<\/strong>
      In oncology treatment, time is a critical factor determining therapeutic success. It encompasses the waiting period for diagnosis, initiation of treatment, length of hospitalization, and the timing of safe discharge. Shortening any of these stages translates into tangible benefits for the patient. A timely discharge in optimal condition contributes to better treatment outcomes and faster recovery.<\/li><\/ul>
      • Financial:<\/strong>
        The current hospital and rehospitalization funding system does not always account for the increased costs of prolonged stays. Retrospective PH\/RH data available to project partners will serve as the basis for developing preliminary \u201ctreatment algorithms\u201d aimed at cost rationalization and more efficient resource use.<\/li><\/ul>
        • Organizational:<\/strong>
          Given the limited availability of hospital beds, effective management of ward throughput is essential. Reducing rehospitalizations and extended stays directly improves service availability and enhances hospital workflow efficiency.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

          RESET will be developed as a package composed of four complementary modules<\/strong> covering the entire continuum of care \u2013 from the first consultation to hospital discharge (\u201cfrom entry to exit doors\u201d).<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

          1. Before admission<\/strong> \u2013 implementation of prehabilitation within the Prehabilitation Clinic.<\/p><\/li>

          2. At admission<\/strong> \u2013 activation of the SAFER Patient Flow Package<\/strong>; enhanced communication between medical staff, the patient, and their family to identify needs, establish a treatment plan, and determine a discharge date.<\/p><\/li>

          3. During hospitalization<\/strong> \u2013 application of the Enhanced Recovery After Surgery (ERAS)<\/strong> protocol; use of the Red2Green<\/strong> tool to assess discharge readiness; inclusion of a clinical pharmacist and social worker in the therapeutic team.<\/p><\/li>

          4. At discharge<\/strong> \u2013 development of an individualized transitional care plan and active monitoring of the risk of rehospitalization (PH\/RH).<\/p><\/li><\/ol>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

            RESET will be developed and piloted in medium-sized regional hospitals operating within the national oncology network.
